Choosing After-Care Services

After-hospital care that fits your needs is important. Make sure you understand what your hospital staff recommends for you or your loved one, whether that's continued medical care like skilled nursing
or hospice care, personal care, or help with activities of daily living. Staff may also recommend where you should receive after-care services, such as in your own home
or at an assisted living
or independent living facility.
If you need a medical device, like a brace
or blood sugar monitor, you can get referrals for those services too. Ask your discharge planner for help finding local after-care services that are right for you.
